New research suggests that birth order may influence the likelihood of developing certain health conditions, with first-borns and second-borns facing distinct risks.

A recent study involving medical data from 10 million siblings in the United States has revealed intriguing links between birth order and various health outcomes. Conducted by researchers from the University of Chicago and Columbia University, the study analyzed information spanning two decades and examined 569 disease categories across families.

The findings indicate that first-born children are more likely to experience neurodevelopmental and psychiatric disorders, including autism, attention-deficit/hyperactivity disorder (ADHD), Tourette’s syndrome, and obsessive-compulsive disorder (OCD). Additionally, they are at a higher risk for immune-related conditions such as food allergies.

In contrast, second-born siblings appear to face different health challenges. The research suggests they are statistically more prone to migraines, shingles, substance use disorders, and gastrointestinal issues like gastritis.

Dr. Marc Siegel, senior medical analyst for Fox News, discussed the implications of the study on the program “Fox & Friends.” He expressed enthusiasm for the research, noting that it raises numerous questions while also providing some insights into the observed trends.

Regarding the increased prevalence of ADHD among first-born children, Siegel proposed a theory related to parental behavior. He suggested that first-borns often receive more intense parental attention, which he described as “hovering” or “helicoptering.” This heightened stimulation, according to Siegel, may contribute to the development of ADHD.

Siegel also addressed the disparities in allergic conditions between siblings, attributing them to early environmental exposure. He posited that first-borns’ immune systems may not be adequately prepared for environmental challenges, leading to a higher likelihood of developing allergies.

Additionally, Siegel considered biological and psychological factors that may influence health outcomes based on birth order. He speculated that the womb environment changes after the birth of the first child, potentially affecting the second child’s development. “The second child gets a womb that’s already been used,” he explained, suggesting this could lead to different health outcomes.

When discussing the higher rates of substance use disorders among second-born children, Siegel suggested that emotional factors might play a role. He theorized that second-borns may seek attention through risky behaviors, driven by a desire to stand out in a family dynamic where the first-born often receives more focus.

While the study highlights significant patterns across a large population, researchers caution that the findings demonstrate correlations rather than direct causation. This means that the observed trends do not necessarily arise from physiological changes, early microbial exposures, or social dynamics alone.

Moreover, the data utilized in the study is derived from commercial health insurance claims, which may reflect variations in diagnosis, healthcare-seeking behavior, and parental attention rather than purely biological factors. Other variables, such as the age of parents at the time of each child’s birth and the age gaps between siblings, further complicate these associations.

Ultimately, the researchers emphasize that birth order serves as a modest population-level risk marker rather than a definitive predictor of health outcomes. The study’s findings were published in the journal Nature Health.
